The program therapist provides direct services to individuals suffering from the disease of chemical dependency, in accordance with all federal, state, and local laws, professional ethics, and Serenity Springs LLC policy and procedure. The program therapist will be expected to provide services in accordance with the Department of Children Families (DCF), Joint Commission on Accreditation of Healthcare Organization (JCAHO), and Commission on Accreditation of Rehabilitation Facilities (CARF) requirements.

The program therapist will meet with each client assigned to their caseload at least once per week for individual therapy as well as do group therapy. In all areas of responsibility, the program therapist will act as an advocate for the client and will work towards empowering the client to move toward self-determination.

Position Available: Shift LPN

General Statement of Responsibilities:
The primary responsibilities of this position are to the Residential Treatment Program while under the supervision of the Director of Nurses. An employee in this position is responsible for the implementation, and evaluation of nursing care, based on the objectives. and policies of practical nursing and the policies of Serenity Springs LLC. 

Specific Areas of Responsibilities:

  1. Read and be familiar with the Serenity Springs LLC summarization of the Federal Confidentiality Regulation, Professional Ethics, Policy and Procedure Manual and the rules and regulations of the Residential Treatment Program.
  2. Report on and off duty as scheduled, clock in and out at the beginning and end of each. Remaining on duty until replacement staff exchange is executed. Notify Director of Nurses (4) hours before your shift, if you cannot come in.
  3. Perform all duties according to program specific guidelines as outlined in Policy Procedure Manual/Standard Operating Procedure:
  4. Attend and participate in staff meetings as required.
  5. Perform the following duties during shift:
    1. Make sure vitals are taken as needed.
    2. Document physician and Nurse Practitioner orders in Zen.
    3. Monitor urinalysis/breathalyzer results and document in Zen.
    4. Call in prescription orders to the pharmacy.
    5. Report all incidents and document in Zen by filing an incident report.
    6. Facilitate medication distribution during scheduled medication times.
    7. Maintain CONFIDENTIALITY of all client care information and to assure client rights are protected.
    8. Report any client leaving the premises without permission or those who have left the premises to the Director of Nurses, Medical Director, Clinical Director, and Director of Operations immediately; document in Zen by filing an incident report.
    9. Report any client abuse immediately (i.e. harsh/abuse language, unnecessary roughness, etc.) to supervisor. An incident report must be filled out documenting the occurrence in Zen.
    10. Chart client adherence to scheduled activities, facility rounds, and incidents in Zen.
    11. Intervene in crisis situations and contact counseling staff and clinical director for guidance/assistance as necessary and required.
    12. Assure completion of shift assignments and tasks prior to reporting off duty.
    13. Monitor medications as prescribed and document in Zen.
    14. Perform other related duties as may become necessary or as instructed by the Director of Nursing and the Executive Team.
    15. Complete agency courses on Ethics, HIPAA, Universal Precautions, and any other training required by DCF/JCAHO/CARF.

Position Available: Recovery Advocate

General Statement of Responsibilities:

The primary responsibilities of this position are to the Residential Treatment Program while under the supervision of the Lead Recovery Advocate, or Director of Operations depending on area of assignment. An employee in this position is responsible for security of the facility and residents, for the resident’s adherence to established schedules and facility rules and regulations and for maintenance of the facility during the shift assignment.

Listed below is an outline of the major duties and responsibilities that the Recovery Advocate (RA) is required to perform. The Recovery Advocate (RA) may be required to perform other related duties and activities, as required. As the RA position incorporates three shifts to provide 27/7 coverage, some duties are not applicable 12AM- 8AM.

Specific Areas of Responsibilities:

Recovery Advocate staff members are responsible to:

  1. Read and be familiar with the Serenity Springs LLC summarization of the Federal Confidentiality Regulation, Professional Ethics, Policy and Procedure Manual and the rules and regulations of the Residential Treatment Program.
  2. Report on and off duty as scheduled, clock in and out at the beginning and end of each shift. Remaining on duty until replacement staff exchange is executed. Notify supervisor (4) hours before your shift, if you are not able to come in.
  3. Perform all duties according to program specific guidelines as outlined in Policy Procedure Manual/Standard Operating Procedure.
  4. Attend and participate in staff meetings as required by supervisor.
    1. Make sure the facilities are kept neat, clean, orderly, and secure at all times.
    2. Check clients in and out of the facility, as necessary.
    3. Monitor urinalysis.
    4. Provide documentation of room searches through annotating in room search log.
    5. Report all incidents and document in Zen by filing incident report.
  5. Assist therapists and case manager in supervising and facilitating client activities through the day.
    1. Implement the behavior management system.
    2. Facilitating medication call, document entries in Zen, transport clients and conduct client locators, as necessary.
  6. Perform all duties assigned in accordance with:
    1. Serenity Springs Admission Program or Residential Treatment Program Policies and Procedures to include:
      1. Verify all clients are wearing appropriate clothing.
      2. Maintain CONFIDENTIALITY of all client care information and to assure client rights are protected.
      3. Report any client leaving the premises without permission or those who have left the premises to the Director of Operations, Director of Nurses, the client’s primary therapist; document in Zen.
      4. Report any client abuse immediately (i.e. harsh/abuse language, unnecessary roughness, etc.) to supervisors. An incident report must be filled out documenting the occurrence.
      5. Supervise client’s meals, medication call, and 12 step attendance. Clients must be in plain sight at all times and staff must keep the group in plain sight.
      6. Receive and refer telephone calls to clinical staff, admission staff as appropriate.
      7. Assist Supervisor with assigned administrative duties.
      8. Chart client adherence to scheduled activities, facility rounds, and incidents in Zen.
      9. Intervene in crisis situations and contact supervisor/therapist/Director of Operations/Director of Nurses for guidance/assistance as necessary and required.
      10. Assure completion of shift assignments and tasks prior to reporting off duty.
      11. Monitor medications as prescribed and document on the medication log sheet.
      12. Monitor client’s activities including their smoke breaks.
    2. Infection Control Procedures:
      1. Wash hands frequently after interaction with client and personal belongings and exercise universal caution.
      2. Check for contraband in client’s possession and report it to supervisor/Director of Operations/Director of Nurses for proper disposition.
      3. Ensure that Universal Precautions are used when conducting UAs, handling spills and cleaning facility.
    3. Safety Rules and Regulations
      1. Keep floors dry and free of unnecessary equipment, personal articles, supplies, etc. to prevent accidents.
      2. Be aware and familiar of ALL Disaster Plans for the facility.
      3. Keep client’s personal articles safely stored and locked.
      4. Report all accidents/incidents, hazardous equipment, or condition, regardless of how minor, to supervisor/Director of Operations as soon as possible and document in Shift Report.
      5. Check all standing lights at night as well as locking of doors.
      6. Report items that need repair to Director of Operations and document in Shift Report.
      7. Supervise client’s visitations, telephone calls and all related communications.
      8. Check bedrooms, kitchen, and commons areas periodically for cleanliness and compliance with rules and regulations. Make facility rounds every 2 hours and document on rounds document.
      9. Advise ALL visitors to sign Confidentiality Sign-In binder by Front Entry into the residence.
    4. Perform other related duties as may become necessary or as instructed by the supervisor/Director of Operations.
    5. Complete all required trainings within the allotted time frame.
